• Registered Voter of Crystal Chapter or if under 18, parents must be registered with Crystal.
• Copy of Applicant’s Certificate of Indian Blood
• Copy of Applicant’s Social Security Card
• Verification of School Enrollment (High School/College)
• Copy of School ID (High School or College)
• Copy of Identification (State ID/Driver License)
• Parental Consent/Worksite Agreement
